SID Feature - Questionnaire Player profile

In addition to the College Prospect and About player profiles, the SID Team Edition also includes a Questionnaire player profile.

The Questionnaire profile is a two page form that an organization can use to collect personal background information for its student athletes.  It is especially suited to sports information directors and their staff's need to maintain player information that can be provided to the media.
